New Patients

We are looking forward to meeting you and are excited that you chose to come and see what we have to offer. Before you come for your first appointment, please download and fill out the attached sheet, and please show up 15 minutes before your appointment so that we can get you in quickly.

What to expect?

When you come to our office, you will be asked to check in at the front desk. Due to Covid 19 restrictions, we are asking that you (if at all possible) bring only one person with you. We understand that sometimes that isn’t possible, especially if you have young children, and we will accommodate as needed, but please do your best to follow the guidelines.

What to bring with you:

  • A valid insurance card and I.D.
  • Your referral if required
  • A current list of medications
  • Your co-payment (we accept all forms of payment: cash, check, credit/debit)
  • If you need xrays, we will take them here at the office but, if you have already had them taken elsewhere, please bring the cd with the images with you.

IMPORTANT POLICIES:

As a new patient, if you miss your appointment without giving us notice, or if you miss repeatedly, we reserve the right to not reschedule you.

Insurances: We take most insurances, however, these are the insurances which we currently do not have contracts with so are unable to take:

Humana medicare, straight medicaid, and magnacare.

We do take all other Blue Cross Blue Shield plans but unfortunately we do not take Blue Cross Blue shield Amerigroup through medicaid.

If you have any of these insurances, or do not have insurance, we do have cash prices for several of our services as well. If you still wish to see us, please call to ask about these.
